A sick leave report is often the starting point of an intensive process involving the employee, employer, occupational health and safety service, and company physician. In this initial phase, crucial information is often missing, even though this is where the most progress can be made. Drawing on more than twenty years of experience in occupational health and safety services, Miranda Zwepink realized that this needed to change. With Triagen, she developed an AI platform that helps professionals enter their first conversation with a sick employee better prepared.

As a case manager, Zwepink observed the same pattern repeat itself year after year. An employee reports sick to the employer, but due to privacy laws, the employer is not allowed to record medical information. The sick leave report is then forwarded to the occupational health and safety service, where additional questions are asked. This is often followed by a consultation with a company physician. Everyone has access to part of the information, but no one has the complete picture from the start.

“The real cause of absenteeism and all the factors involved often don’t become clear until much later. Yet it is precisely in that initial phase that there is the greatest opportunity to provide the right support,” says Zwepink. Together with co-founders Danny Hoendervangers and Stefan Samba, she explored a way to uncover the right information earlier. The problem, rather than the technology, was their starting point. This approach led to the creation of Triagen.

AI collects information; the professional conducts the conversation

Triagen kicks in immediately after an employee reports being sick. Through a secure environment, the employee can explain what’s going on at a time of their choosing. The system automatically asks follow-up questions about topics relevant to the situation, such as work, personal circumstances, or other factors that may influence the absence.

It does not use a fixed questionnaire. AI adapts follow-up questions based on the answers a person provides. For example, if someone has back pain, the system asks follow-up questions on relevant topics and skips irrelevant ones. This creates a more complete picture before a professional conducts the initial interview.

Behind the scenes, Triagen clearly summarizes all the information and highlights key points of concern. This allows a case manager or company physician to more quickly determine the most appropriate next step: providing support themselves, referring the person directly to a company physician, or, for example, initiating an intervention such as physical therapy.

Better preparation leads to better support

According to Zwepink, the greatest benefit lies not only in time savings but, above all, in the quality of service. “In current practice, information comes from all sorts of sources: an absenteeism tracking system, emails, phone calls, and various reports from colleagues. That’s fragmented. Triage brings that information together in a structured way from the very beginning.”

As a result, professionals have a complete overview sooner and can provide the right support more quickly. Furthermore, it creates greater uniformity in the provision of information, which makes case files more consistent and handoffs smoother.

AI supports, but does not make medical decisions

Although AI plays a central role within Triagen, the human professional always remains responsible for the assessment. “The system does not provide medical advice or make diagnoses,” emphasizes Zwepink. “AI collects information, creates a summary, and helps structure the conversation. The professional then determines for themselves what actions are necessary.”

Triagen considers this principle to be important. AI should never take over the role of the professional. Occupational health and safety services are all about trust, personal contact, and tailored solutions. By automating the preparatory work, more time is freed up for the conversation that really matters. “Instead of wasting valuable time gathering basic information, the professional can focus on the employee and work together to determine what is needed to return to work sustainably.”

With Triagen, Miranda demonstrates that AI tools add the most value when they arise from a real-world problem. By making information available sooner and better preparing professionals, the platform contributes to faster, more informed support for employees on sick leave.

Want to learn more?

Curious about how Triagen supports occupational health and safety services with AI-driven triage and consultation preparation?
